  • Please delete any pre-entered information in the “Special Instructions” box.
  • Click on “Submit Request”
  • Click on “Make Payment”
  • Complete the payment page and click “Submit”

Re-Evaluation

To add new documents or change the type of a completed report, a new application and full fee must be submitted. Please log in using existing account information. Then click “New Application” at the top of the page.

Review After 90 Days

Requests for review of the evaluation, using documents received with the original application, must be made in writing within 90 days of receiving the report.  Requests for review must be submitted in writing either via postal service or email. Please include your reference number and be very specific in your concerns.

  • Click on “Make Payment”
  • Complete the payment page and click “Submit”
  • Send email with reference number and specific review request to gce@gceus.com
