APPLICATION FOR EVALUATION OF EDUCATIONAL CREDENTIALS

INSTRUCTIONS

  1. Please make sure you read and understand the information in the Types of Evaluations Guidelines sections of the website before you fill out this application form.
  2. You can download the application in MS Word Format or Adobe PDF format. You will need the free Adobe PDF reader for this.

  3. You may also send an e-mail to request a faxed or mailed copy of the application.

  4. If you need copies please use this form or you can order a copy online.

  5. Please use this applicaiton to request a word for word Translation of documents not in English.

APPLICATION PROCEDURES

Persons requesting an evaluation report must submit ALL of the following in one packet

  1. Application form, completed in full.

  2. Clear and legible photocopies of all official educational documents (diplomas, certificates, transcripts, mark sheets, grade reports, etc. indicating all subjects taken and credit hours earned), beginning with the final year of secondary school. If you plan on attending TEXAS SOUTHERN UNIVERSITY you will need to have the university you attended send your educational documents directly to our office.

  3. Complete, literal word-for-word, certified English translations of all foreign language documents. We can provide this service for you. Please contact us for more information.

  4. Payment by credit card, money order or cashiers check for the correct amount made payable to Global Credential Evaluators, Inc.

FOR A DOCUMENT-BY-DOCUMENT EVALUATION degrees, diplomas, or titles are required. Transcripts, marksheets, examination results, grade reports, etc. are useful.

FOR A COURSE-BY-COURSE EVALUATION degrees, diplomas, certificates, titles, transcripts, marksheets, grade reports, etc. indicating all subjects taken and hours.

To be considered for admission into a university in the United States: Applicants to the first year of university or college should submit all four years of secondary and upper secondary school.

Applicants wishing to transfer to a U.S. university should submit secondary, upper secondary and all post secondary (university) academic records.

Graduate applicants should submit all university records. For other purposes requiring a course-by course evaluation (such as teacher certification, and professional licensing) all post secondary records are required including diplomas or titles.
